By submitting this application, I agree to the rules and regulations
of Wyoming Seminary. I hereby consent and authorize Wyoming Seminary
to photograph and/or videotape me, together with any subject matter
owned by me, and authorize the school to use such photographs, (including
sound, motion pictures, and/or any other form of picture recording,
radio, television, or print media) for all purposes which it deems proper.
I also authorize the school to release information to the public which
it deems proper for publicity purposes. I do hereby release Wyoming
Seminary from any and all claims for damages, libel, slander, invasion
of privacy or for any other claim based on the use of said material.
I am filling this form out with the consent and knowledge of my parent
or guardian.
Recommendation Forms: In addition to the application, you should submit two recommendation forms completed by a classroom teacher and a private instructor of music or dance. These recommendations may be filled out online or be mailed separately.
Audition Requirements: Students may audition in one of two ways - by submitting recorded material, or by live audition. Audition requirements vary by program. Complete audition information is located on the print version of the PAI application, on the admissions page of the PAI Web site or by contacting the PAI office.
Application Fee: Please submit the application fee of $35.00 until May 1, 2008 or $50.00 after May 1, 2008, payable to Wyoming Seminary.
