EMPLOYMENT INFORMATION

Each case is considered on its merits. You may omit minor traffic violations, offenses committed before your 18th birthday, which were adjudicated in juvenile court or under a Youth Offender Law, and any convictions which have been expunged by a court.             In recognition that work with and around youth involves a great deal of responsibility, I certify that all information is true and complete, and subject to verification. I understand that any omission, misrepresentation, or falsification of information provided on this application or subsequently by me or by another in my behalf will be cause for rejection of this application or for subsequent discipline up to and including dismissal from employment.

            I also understand that if my application for employment is accepted, the effective date of my employment shall be the time I actually begin to work. I further understand that my employment is not guaranteed for any term, and that my employment may be terminated by the School or me for any reason. I further understand that if employed, my employment will be subject to the conditions of any applicable initial evaluation period. I further understand that my employment at the School is “at will.” This means that I am free to resign from employment at any time. Similarly, the School is free to terminate the employment relationship at any time, with or without cause or prior notice, as it deems appropriate.

            I further understand that proof of employment eligibility will be required within three (3) days of the date of hire. I understand that if I am hired, this form will be transferred to my individual personnel file.
