Is the student registered as an UNACCOMPANIED MINOR?*

Many airlines require young adults under the age of 15 to register as an "unaccompanied minor" and provide additional information about the person picking him/her up at the airport. Airline policies vary, so please verify your airline's unaccompanied minor policies in advance. We will provide all necessary contact information no later than one week prior to the start of the program.

The student is NOT registered as an unaccompanied minor. The student IS registered, or plans to register, as an unaccompanied minor. I understand that it is my responsibility to communicate this and the necessary contact information to the airline. Failure to do so may result in travel complications.

I understand that I must notify Summer at Sem of any changes in travel plans, including flight changes and cancellations.
